That's one important difference to stay aware of: Not always people desire to live all of their fantasies.
I do have my share of dark fantasies as well, but they are purely a "mind game".
If somebody feels a real desire to do substantial harm (my definition being: unrepairable damage) to others, considering professional help may be worth some thought.
Fantasy, on the other hand, doesn't need limits...
