From the picture it is not clear but maybe there is a small motor with an eccentric moving the dildo up and down like in the chinese donkey...
you know, fucking machines were invented long ago !!! The oldest I found is a steam operated one (1891) and in the 8 mm loops of the Seventies they were already popular !
