I have been really diving deep on the how what and why of this fetish lately.

Thought-provoking discussion. I think you are on the right lines when you suggest it's about power and control - either having it or being subjected to it. And some of us like to fantasise about it being taken to the ultimate level. But this is something that I find hard to theorise about too much, given that women suffering and dying has been something that turned me on before I even knew that's what was happening to me!

It seems to me that the biological aspect is indeed very important. Women need some kind of passive/submissive attitude in order to enjoy being penetrated, which easily goes towards masochism. And men need some aggressive/dominant trait in order to be able to penetrate a woman, which goes intrinsically towards sadism. Such primers come with the mechanics of sex itself.
But if this is the whole story, it cannot explain why there are also masochistic men, and why sadism and masochism is so intimately related. One early finds one without at least traces of the other in a person. So maybe one has to add something more from beyond pure sexual drives to it. A possibility comes from the observation that human behaviour, even beyond sexuality, is too complex to be explained by simply seeking pleasure and avoiding discomfort. Maybe some alcoholics or gambling addicts show what happens if this death drive takes over and eventually results in physical annihilation.
But this drive could then nicely interact with the sexual drives. I’m unsure if Freud delved this deeply into kink. (I only did one year of Psych and didn’t delve deeply into his studies of sexual deviancy as I believe he called it).
