It`s called a Wartenburg wheel , if you want to purchase one go to a leather suppliers and buy a wheel suitable for cutting holes in leather- same thing.I would not suggest pressing too hard on the nipple though you might end up with a series of holes . The idea is to gently run it over the skin preferably the sensitive areas
