I think the time of inquisition was a very dark and mad time. "Witches" were destined to die. After a few days (or even weeks) of torture this poor women have surely not been "playthings" any more. So it didn´t matter when their intimate parts where destroyed.
But as with each instrument, it depends on the use. Pears can - like whips - be used without destroying.

More seriously, it's very likely that the sadists among Inquisitors drew pleasure and sexual satisfaction (only) in inflicting terror and pain on the victims. They didn't really want to (or maybe they couldn't) have sex with them.
