Those XIX century artists were trying to depict something they had never actually seen. Victor Hugo hadn't seen "the boot" in use either. If I recall, his description is vague. He seems to describe it enclosing only one leg or one foot and being operated by a screw mechanism.
That's the way they show it in the 1982 version, starring Leslie-Anne Down.
A lost film from 1911 seems to have the boot enclose both legs and have Esmeralda on the rack.
Most contemporary descriptions are a set of planks that squeeze both legs with wedges. The 1922 Danish film "Häxan" did have a demonstration of this type of boot.
