Going through the examples of how my "clients" get attached, there's one constant. It's almost always ropes
plus nails. I see Hasturan does that too. That setup gives me the freedom to place the nail wherever I want to. No worry about whether it'll really hold the weight or not. If you put the nail sideways through the ankle or heel, obviously you'll crush or penetrate bones.
This is about as close as I get to the "classic" style.

And of course the scene I did for Apostate's story. The story dictates that there's no additional ropes, the nails go through the wrists, and victims are shaved. So that's what you get.
For the arms, it's a mix in my drawings whether the nails go through the wrist or palm. Historically, and for supporting the cruxee, palm is certainly wrong, but I sometimes do that anyway.

and palm! Maybe that was an apprentice carnifex who got told off, that's not the way to do it, put in another nail!
I just think that the nail twisting among the nerves if it goes right through the wrist might be so godawfully painful that the crux-girl wouldn't be dancing up a storm. So sometimes I put it through the palm if I'm either feeling merciful or want the girl to dance a lot.
More importantly, I don't always want to do crux exactly the same way.
For purpose of sheer provocation, some aren't nailed at all, or so delicately that rescue would be easy. Not that we would do that
