I'm not sure that's the safest way to hold a pepper-box pistol. Especially since they tended to fire all cylinders at once. However, it does look like it would be difficult to hold with one hand. Another drawback of the pepper-box was that it was mussel heavy and hard to aim.
That's Amber Heard form the 2011 movie "Drive Angry". Considering the movie is set in 2011 and the last pepper-box was manufactured before the US Civil War; I'm wondering why she has one in the first place. I haven't seen the movie. It's a Nicholas Cage action film so I probably won't bother seeing it.
