According to the Rule of St Benedict, monks were (and for all I know still are) only issued with 'femoralia', underwear, if they were going out of the monastery on an errand. 'Femoralia', literally 'thigh-wear' would have been a loincloth in his time, around 500. Nuns following the Benedictine Rule would probably have observed the same practice - except they were less likely to go out of the enclosure, and of course St Ben omitted to say anything about bras, or in those days breast-bands.
