The only limit is the creativity of the executioner. Ultimately, you don't get to choose. Your carnifex does. And you might hope that he picks your ideal position, but he will almost certainly pick the most painful pose he can...

Well for the foot/heel nails there are a ton of options that were skipped. Footrests, nailing the feet side-saddle, and how high or low you nail the feet just off othe top of my head. For the wrists/hands, there is also the matter of rope, wooden washers, and nail type in addition to the exact position of the nails

Are there styles like backwards, upside down, spread eagle or combinations of them?

There is no limit to the variations

If the executioner puts you on the X-cross, it's easier to pee and you probably die faster. Also, they don't get to nail your wrists and then drag you to the stipes. Plus you don't have anything to carry to your execution (unless they make you carry the whole X-cross, but those are heavy). So it's a mercy, really!
