If you watch the video, it's clear she's not acting. That cattle prod contraption they use on her must hurt.
Yes, her body language, expressions and screams all together didn't look faked at all. Not only did it hurt, but she was surprised by how much it hurt. I used the soundtrack from that for a short animation I did a long time ago, one of the old versions of Sabina struggling on the cross, mixed Cowgirl's screams with the crowd noise background from The Passion of the Christ crucifixion scene. It was an interesting exercise, making Sabina's mouth shape and movement fit with Cowgirl's screams.

Okay! I am knit picking.
Because they want her hanging by her wrists struggling so people can see that she is properly punished, and so they can see where her feet should be positioned on the cross. Not too high, but not stretched out too low either, high enough so she can get her feet under her and push up as far as she can. Get it wrong and she might not last!
