As you can see, if the nail was placed between the second and third Metatarsal just below the intermediate and lateral cuneiform, no bones would be broken.

I would think that they just used their fingers to feel for depressions and separations. ;)

It takes strong, supple, experienced fingers, though, doesn't it? Fingers that know how to poke and probe, and can find that sweet spot.;)
 
That x-ray is taken with the foot at an angle. With the foot flat on the cross you'd need to angle the nail in about 25 degrees to go through that gap.
